Service Agreement

Please review our Service Agreement. This Service Agreement outlines the terms and conditions for services provided by The Lice House. By scheduling an appointment, submitting an intake form, or receiving services from The Lice House, you acknowledge that you have read, understood, and agreed to the terms outlined below.

1. Appointment Confirmation & Card on File

• A valid credit or debit card is required to schedule any appointment.

• Your card will be securely stored but not charged unless:

1. services are completed, or

2. a cancellation/no-show fee applies.

2. No-Show & Late Cancellation Policy

To respect our technicians’ time and ensure availability for all clients:

• No-Call/No-Show Fee: A fee of $75 will be charged to the card on file if you fail to show for a scheduled appointment without notice.

• Late Cancellation: Cancellations must be made prior to the start of your appointment. For same-day bookings, we require as much notice as possible to avoid a $75 late cancellation fee.

• We understand emergencies happen — please communicate promptly so we can accommodate your family or rebook your slot.

3. Payment for Services

• Full payment is due upon completion of all services performed during your appointment.

• If additional treatments or individuals are added at the time of service, charges will be updated accordingly.

4. Accuracy of Information

To properly estimate appointment length, staffing, and costs, you agree that all information provided in this intake form is truthful and accurate to the best of your knowledge.

Final pricing is determined after an in-person evaluation.

5. Treatment Expectations & Limitations

• Upon arrival, our technician will conduct a thorough lice check to confirm findings.

• The number of treatments, duration, and costs may vary depending on hair type, infestation level, and number of individuals being treated.

• You acknowledge that results depend on adherence to our after-care instructions.

6. Parent/Guardian Consent

By booking, you confirm that you are the parent or legal guardian of any minor receiving services or that you have obtained proper consent.

7. Rescheduling

• Rescheduling is allowed at any time prior to your appointment.

• For same-day appointments, please contact us as soon as possible if you need to reschedule to avoid any fees.

8. Illness Policy

For the health and safety of all families, please reschedule if anyone in your household is experiencing fever, vomiting, or other contagious illness. No fees will apply in these cases as long as you notify us promptly.
